Our company don't fire anyone for one positive test. The person is given the choice of going through a company approve treatment program at the employees cost, and subsequent weekly testing for 6 weeks, and then bi-monthly testing for one year. All of that is at the employees expense.
Try being a stoner in the nuclear industry...hell, ANYWHERE in the utility industry.
And it's one of those little things, along with sexual harassment, racial comments, violating safety regs, etc., where the union will step aside and say, "He's all yours."

And Issa, bleach is where someone will try to "contaminate" the sample by using bleach. Most places view that as a positive test, much as they do for other agents meant to "hide" drug usage such as Golden Seal, etc.
