Ezra Klien is a noted liberal and obumbles ass kisser. He could quite possibly be correct about some of what he talked about, and to me that is the problem. The US is largely funding the drug companies R&D and the rest of the world is reaping the benefits. Personally I think these drug companies ought to tell the rest of world... no payee, no live saving drugs. That, in my opinion, is the problem. 
Well, this method of wealth redistribution to second and third world countries will just have to do until 'climate change' methods can be foisted on the general public.
