A State Department-ordered investigation into September's attack on the U.S. Consulate in Benghazi, Libya, concluded that "systemic failures" left the facility inadequately protected, according to the independent review board's report, which confirmed that no protest preceded the deadly attack.

The report, posted Tuesday night on the State Department's website, also identified "leadership and management deficiencies at senior levels within two bureaus." It suggested 29 ways the department can improve its operations, but recommended no disciplinary action.

The Accountability Review Board's report comes after more than three months of intense debate in Washington over who was behind the attack, what motivated the attackers and why U.S. authorities weren't able to stop the violence, which took the lives of Ambassador Christopher Stevens and three other Americans.

Republicans have accused Obama administration officials of giving the American people a series of misleading explanations for the attack from the start. Much of the criticism focused on U.N. Ambassador Susan Rice's comments five days after the Sept. 11 attack that the violence was a "spontaneous" result of protests against an anti-Islam film. She later backtracked, saying her comments were based on the most current information provided by the intelligence community.

Although the motive for the attack remains unclear, the report released Tuesday confirms what quickly became evident -- that the attack was the coordinated work of heavily armed terrorists.

The report confirmed that the attack involved "arson, small arms and machine gun fire, and the use of RPGs, grenades, and mortars."

The security failures, though, were singled out repeatedly in the report.

Despite clear and present threats, the review board found the security staffing at the Benghazi consulate was "short-term, transitory" and "relatively inexperienced" -- and ultimately "inadequate."

It also found "a pervasive realization among personnel who served in Benghazi that the Special Mission was not a high priority for Washington when it came to security-related requests, especially those relating to staffing."

Clinton vowed Tuesday to address the deficiencies identified in the report.

"The Accountability Review Board report provides a clear-eyed look at serious, systemic challenges that we have already begun to fix. I am grateful for its recommendations for how we can reduce the chances of this kind of tragedy happening again. I accept every one of them," Clinton said in letters to the Senate and House panels reviewing the attack.

Although much of the focus is on the State Department's preparedness, the review board didn't let Libyans off the hook. Some of the security was provided by a militia, and there were "some troubling indicators of its reliability in the months and weeks preceding the September attacks," the report said.

The review board "found little evidence" that the guards provided by the militia "offered any meaningful defense" of the compound.

Three months after the worst terrorist attack on an American diplomatic outpost since the 1998 embassy bombings—and the most brazen terror strike on U.S. interests since 9/11/2001—the State Department this week will try to clarify what happened in Benghazi this past September 11.

Congress will be briefed Wednesday by an independent advisory board created by State to report on security in Libya. Two deputy secretaries of state will testify Thursday in open hearings in the House and Senate—a few steps up the pay grade after career civil servants were sent to take the heat in October. Fuller disclosure on Benghazi can set the record straight and help draw security and policy lessons.

But the critical piece of this puzzle will once again be missing. Secretary of State Hillary Clinton was scheduled to testify in Thursday's open hearings. She came down with a stomach virus last week, cancelling a foreign trip.

On Saturday, as the media were consumed by the Newtown massacre, a Clinton spokesman released a statement saying that earlier in the week Mrs. Clinton had fainted and sustained a concussion. On the advice of her doctors, she won't be able to appear on the Hill. The statement didn't explain why her injury wasn't made public earlier.

Health comes first, and we wish Mrs. Clinton a speedy recovery. In a letter to House Foreign Affairs Chairman Ileana Ros-Lehtinen, Mrs. Clinton sent her regrets and said she looks forward "to engaging your committee in January so that we can address the tragic events that occurred in Libya and the measures that the State Department is taking in response." Mrs. Clinton copied Senate Foreign Relations Chairman John Kerry.

By January, however, Mrs. Clinton's likely successor (Senator Kerry) will be nominated and probably on his way to confirmation. Hill sources say it'll be hard to schedule another hearing and call her back. Congress should insist on it.

Mrs. Clinton's testimony is months overdue. Ambassador Chris Stevens and the Benghazi consulate staff reported to her. Their safety was her responsibility. Congress needs to flesh out why security was so lacking, why requests for additional protection for the mission were denied, and who made those decisions.

Despite background briefings by the Pentagon, State and CIA, the Obama Administration hasn't offered a consistent timeline of the Benghazi events. Mrs. Clinton hasn't said what she did that day and precisely how her department liaised with the military and intelligence services. It shouldn't take this long to fill such gaps.

The backdrop to Benghazi matters too. Mrs. Clinton was presumably—as the President's chief foreign policy adviser—instrumental in drawing up the "light footprint" policy in Libya. After the fall of Moammar Gadhafi, the U.S. disengaged. As an elected but weak government struggled to establish itself in Libya, Islamist militias with al Qaeda ties filled the gap. One such group, Ansar al-Shariah, laid siege to the U.S. consulate and CIA annex in Benghazi, killing four Americans. Both the CIA and State immediately pulled out of the city—an abject retreat. What was the rationale for the U.S. approach to Libya, and will it change?

Mrs. Clinton will soon leave the Obama cabinet with sky-high approval ratings and an eye on the 2016 presidential nomination. It's logical for her not to want to dwell on the worst debacle of her tenure at State. But two months ago, she said "I take responsibility" for Libya without ever doing so. It's well past time she did.

I wonder if the National Enquirer is right (yet again vs. MSM!) and Hillary really DOES have brain cancer.  If so, SHE may have a valid excuse for not having exercised due diligence in the performance of her duties.  However, as we all know, the President is the main driver in our reduced national security across the board. :mad:  He'll never have to testify before Congress under oath though, because there aren't enough Republicans with cojones to force the issue.  Most of them are neutered, and of course the Dems are slavering fools over teh Won. :argh: :argh: :argh: :argh:

I hear ya, Boudie, but it's a loooooong stretch for me to look at that rag at the supermarket checkout stand and think "quality journalism." A reaaaaallllly long stretch.

I'd say first priority for Hillary after she "retires" as SecState is to form her committee and start collecting money for 2016. She'll do it quietly and her fawning masses won't hear or see much from her in a year or more, but then the power-hungry whore she is will come into play and she'll write another book and do another book tour to add to her personal coffers.
