It was so obvious what caused it--instead of taking it slow and easy, the liver spotted old stumble bum is always trying to put on a show of running or jogging in an attempt to look young, athletic, and vigorous.  When he turned to exit, you can see how he raised his forearms up into what he must think of as a running stance.

that's the first thing I thought when I saw it.. he was trying to do his famous geezer bounce-n-shuffle instead of looking where the hell he was going.

And to top it all off, when he got back to the White House he promptly banged his head on the header of the door on Marine One and did his creepy Frankenstein walk across the lawn.  Then he sloughed off his fall with his stupid joke (that he seemed to think was clever but someone probably fed to him, anyway) about getting sandbagged.  Then, unbelievably, he did his stupid little shuffle jog again.  He is tempting fate and one day he is going to do it one time too many.
