Dehydration
might have been
a factor (though none are seen in the pic, even in the 1970s I cannot
imagine Carter and his Secret Service agents not having had water bottles, probably dropped when Carter started to show signs of extreme exhaustion). The main causes, however, were heat and,
most importantly, Carter was attempting a very challenging 10K (6.22 miles) for which he was under-prepared. The "optics" were horrible, but I won't belittle him, except a bit for poor judgment (and BTDTGTTS).
