Nothing special, just a ubiquitous part of working in a government/military office.

They are rather odd though because if you've never seen one they list various attributes to be noted while speaking with the person phoning in the threat and they are arranged as easy to mark check boxes.

These are not the cards we use but a fair approximation:



Which strikes me odd as the whole thing rather reminds me of ordering sushi:

We had a computer store down in Melbourne that worked like a Chinese take out. You picked up the sheet with the current parts they had in stock, checked off your selections and took it to the counter where the non-english speaking staff quickly filled your order. It was totaled up by the old grandmotherly type who spoke some broken engrish while taking your money.

It was very efficient and very cheap. It worked quite well. They made enough money to build a spiffy new building that was 3 times the size of their old pizza hut building. I haven't been by there in years, they may still be in business.

We had the same thing  while I was USN and started standing Assistant Squadron Duty Officer Watches (Typically senior PO2s [E-5] or above at the time. I was a Junior PO2 [E-5] when I started standing those watches) That was back in 1979.

I got cards like that. And just like yours, mine ask for the bomber's name and address too.
Always wanted to say, "I'm sorry sir, but you can't explode until I have your name and address."

I was on the evacuation committee for my work, and we learned a bit about threats. Bomb threats will not automatically evacuate the building. That's because we get 150+ bomb threats a day. Something's really got to stand out for it to be taken seriously.

and ask ALL those questions. apparently more than a few gave their name and address before they realized what they were doing.
