The Obama administration received clear notice more than five years ago that VA medical facilities were reporting inaccurate waiting times and experiencing scheduling failures that threatened to deny veterans timely health care — problems that have turned into a growing scandal.
Veterans Affairs officials warned the Obama-Biden transition team in the weeks after the 2008 presidential election that the department shouldn’t trust the wait times that its facilities were reporting.
“This is not only a data integrity issue in which [Veterans Health Administration] reports unreliable performance data; it affects quality of care by delaying — and potentially denying — deserving veterans timely care,†the officials wrote.
The briefing materials, obtained by The Washington Times through the Freedom of Information Act, make clear that the problems existed well before Mr. Obama took office, dating back at least to the Bush administration. But the materials raise questions about what actions the department took since 2009 to remedy the problems.
In recent months, reports have surfaced about secret wait lists at facilities across the country and, in the case of a Phoenix VA facility, accusations that officials cooked the books to try to hide long wait times. Some families said veterans died while on a secret wait list at the Phoenix facility.

The Project on Government Oversight (POGO) and the Iraq and Afghanistan Veterans of America unveiled an encrypted web submission form Thursday soliciting horror stories in the wake of a nationwide furor about fudged wait time records and related veteran deaths in Phoenix.
POGO Director of Communications Joe Newman says the groups are looking for systematic problems and received 310 submissions as of Monday morning. The majority of reports are thus far from veterans and members of the public, and less than 10 percent are from VA employees, Newman says.

MIAMI (CBSMiami) – When asked why he would risk his job and speak publicly, Detective Thomas Fiore considered the question carefully before answering.
“People are dying,†he finally said, “and there are so many things that are going on there that people need to know about.â€
Fiore, a criminal investigator for the VA police department in South Florida, contacted CBS4 News hoping to shed light on what he considers a culture of cover-ups and bureaucratic neglect. Among his charges: Drug dealing on the hospital grounds is a daily occurrence.
“Anything from your standard prescription drugs like OxyContin, Vicodin, Percocet, and of course marijuana, cocaine, heroin, I’ve come across them all,†he explained.
Even inside the hospital, he says he was stopped from doing his job – investigating reports of missing drugs from the VA pharmacy. When the amount of a particular drug inside the pharmacy doesn’t match the amount that the pharmacy is supposed to have, a report, known as a “discrepancy report†is generated. Normally it was his job to investigate the reports to determine if they were the result of harmless mistakes or criminal activity. But all that changed, he said, about two years ago.
“I was instructed that I was to stop conducting investigations pertaining to controlled substance discrepancies,†he recalled.
He said he was personally told to stop investigating them by the hospital’s chief of staff, Dr. Vincent DeGennaro.
“I have no idea why,†he said. “He’s the chief of staff he doesn’t have to tell me why.â€

The director of the Phoenix VA hospital where 40 veterans died while waiting for care received an $8,500 bonus last month, according to Jeff Miller, chairman of the House Veterans Affairs Committee.
Sharon Helman, the director of the Phoenix Veterans Affairs Health Care System, "got an $8,500 bonus last month while there was an open [inspector general] investigation into Phoenix," Chairman Miller told CNN's Jake Tapper in an interview Wednesday.
It had been previously reported that Helman received more than $9,000 in bonus pay in 2013 on top of her annual salary of $169,900. The VA office of inspector general began investigating the Phoenix VA for wrongdoing in December 2013, months before Helman received the additional $8,500 bonus.
Update: The VA announced this afternoon: “Secretary Shinseki today exercised his authority to rescind Sharon Helman’s fiscal year 2013 performance award immediately. Previously, Ms. Helman received the performance award due to an administrative error.â€

A VA police officer says administrators at the hospital in Miami where he works are covering up crimes at the facility, including evidence of physical abuse of patients and drug dealing.
Fiore earlier told the Miami Herald that the breaking point for him was a March 2013 report on the facility’s residential drug rehab program, which charged the program failed to adequately monitor patients or stop illicit drug use.
The report highlighted Nicholas Todd Cutter, a 27-year-old Iraq war veteran who overdosed on drugs shortly before he was set to graduate the program. Fiore told the Herald Cutter’s death “could have been prevented.â€
“I was told that the police reports were to stop,’’ he said, “and they would notify me if something important came up.’’

By now most American have heard about the VA’s infamous patient “secret wait lists†which reportedly contributed to the deaths of up to 40 veterans in the Phoenix area alone. Those patriots were American heroes who served our country proudly. Yet they were left to die waiting to see a doctor.
Twenty-three employees at a Department of Veterans Affairs center in Louisiana were placed on leave in 2010 as part of an investigation into document forgeries, a move revealed only in federal whistleblower lawsuits filed years later.
The investigation focused on records required to prove staff “competencies†at a medical facility in Shreveport, Louisiana, according to the complaints.
Nancy Faulk, an associate chief nurse at a VA hospital in Shreveport, filed a lawsuit last fall disclosing that employees “were identified as participating in the falsification of competency records.â€
She accused the department of retaliating against her for raising concerns that race figured into disciplinary actions against three black nurses as a result of the investigation, according to the lawsuit, which is pending in federal court.
LOS ANGELES (CBSLA.com) — The bodies of 28 veterans at the L.A. County Morgue were finally moved Friday for burial to the Riverside National Cemetery.
CBS2/KCAL9 pressed the L.A. County Coroner’s Office Thursday to find out why the bodies had not yet received a proper burial after a source indicated there may have been as many as 60 veterans at the morgue for the past year and a half.
The morgue says the bodies were unclaimed and they don’t know how long the veterans were there.
The law states veterans are supposed to receive a proper burial.
