In May, my wife and I will be celebrating our 10-year anniversary. I was thinking it would be fun to surprise her with a vow renewal ceremony. Has anyone ever done one of them before? How should I go about planning it?

How would I know? I haven't worn out the last set yet.
Seriously, ask a preacher or the scatter brain touchy-feely woman at work. Personally, I'd go with the pracher for a start.

My Ex and I did informal ones at 5 & 10 yrs,then did a formal one at 20. Course the 20 yr one actual tookplace on our anniversary. At 5 & 10 I was predisposed so we did them later.

I second Johnny's suggestion. Mr Smith is a preacher guy...when we got married, he thought of stuff I'd never even considered. He did the same thing when he married my oldest kid and wife. He has all the different vows, ceremonies, etc. Call a/your preacher, he'll know it all.

I tried that with my wife at ten years and she told me that she just wanted to stay friends. :(

Dude -- you don't have to be romantic anymore to score :)
(OK, I wish I had thought of it. Womern love that a guy thinks of thems and does this kind of thing. YOU CAN NOT DO WRONG -- the idea itself is enough. If you spend a few thousand and do a church thing, she will remember it forever. Tuck her wedding dress in a box in the back/hood of the family car, have your tux ready, find an excuse to go to the nearby church, then spring it).
You're a good man. And you have set my wheels a'thinking -- Mrs. FD (15 years!) will be here next month and it would be cool if I could re-wed her in a chapel in Williamsburg.

Do you know why they ask that? It wasn't for a dramatic Ben Stiller moment -- it was to determine whether anyone knew if the couple was related by blood (1st or 2nd cousins). Many times, only the wedding attendees knew if the couple were related. The parson was asking "are these 2 related?" Not "is there an asshole present?"
Great trivia, yes?
