Looks like the entire cylinder, not just the piston, failed. Holy crap -- that thing went off like a bomb.

I am a Locomotive Engineer by trade. The same thing happened to me, sounded like a cannon going off, threw the piston through the car body into the roof of an old freight house. Had a heck of a time trying to get the dispatcher to believe me.
