Not only was it racist, and very nasty, but it was incorrect, too.

Democrats take for granted minority voters and anyone who gets off their plantation (or was never on it to begin with) must be shamed and destroyed. It’s sick. So, realizing how nasty and ugly the cartoon was toward Vivek, an apology was issued by the Iowa newspaper’s executive editor, Tom Martin.
The "apology" is all TL;DR blah-blah and no teeth - i.e. no editorial staff member making the decision to publish was disciplined.
The cartoon was a two-fer, portraying Ramaswamy as puppet for bigots and R rally attendees as ignorant bigots. I doubt that anyone, at this point, reasonably thinks Ramaswamy has much of a chance
in 2024, but he might be a threat to MSM/Dem stereotypes in future years.
The form will vary, but every possibly significant R candidate will experience continual crap like this from now until Election Day 2024, and for another 4+ years if they dare to win the Presidency.
