Gen. David Petraeus, the top U.S. commander in Afghanistan, apologized to the Afghan government on Wednesday for a helicopter attack that killed nine children, and he plans to personally tell President Hamid Karzai he's sorry.

“We are deeply sorry for this tragedy and apologize to the members of the Afghan government, the people of Afghanistan and most importantly, the surviving family members of those killed by our actions,” Petraeus said. “These deaths should have never happened.”

A report said the attack in Kunar province on Tuesday was caused by an “error” in identifying the location of insurgents.
